Effective Recording Skills - Training for Care Staff

Effective recording is an essential part of service delivery in social care. Every individual, who works with vulnerable people, should be clear about the principles and purpose of record keeping. 

This one-day course provides participants with the principles of good practice and is delivered in line with policies and legislation.

It is important to appreciate effective 
recording maintains a history for the person, and should highlight any issues or developmental needs, and provides continuity when key workers change.  

Points covered in this course will include developing skills of recording, understanding how to 
analyse patterns of behaviour, as well as being able to observe, conceptualise and make action plans.

Course objectives:
  • Develop confidence and skills to enable participants to record accurately.
  • Understand that it is essential to distinguish between ‘facts’ and ‘professional judgements’,  hypotheses, non – factual information and conclusions based on observation and assessment.

  • Provide clarity about the purpose ofrecording and for whom it is being written, working from the principle of ‘openrecording’, which would enable people to see and contribute to their records wherever appropriate and possible.
  • Assist participants when structuring records and address how to focus on key themes and issues. 
  •  Address key principles around information sharing both within and across agencies.
  • Help participants to understand the importance of recording in terms of analysing patterns of behaviour, improvements, problems and needs.

By the end of the course, participants must be able to understand:

  • The purpose and function of recording
  • Why recording needs to be executed in a professional and systematic way
  • How effective recording benefits the worker, the service and the service user
  • The importance of using evidence and being able to reference it when recording   
  • The potential consequences of poor recording
  • The importance of using simple English
  • What is meant by ‘objective’ recording; dealing with our own:
                 -Bias
                 -Prejudice
                 -Assumptions
                 -Stereotyping etc
  • Distinguishing between fact and opinion

  • The use of non-judgmental and anti-discriminatory language

  • How jargon and abbreviations can alienate people
