This course introduces person centred planning, helping learners understand how to place individuals at the heart of care and support by recognising their preferences, strengths, and goals to promote choice, independence, and wellbeing.

Learning Outcomes

The Person Centred Planning course supports learners in developing a clear understanding of the principles that underpin Essential Lifestyle Planning and Person-Centred Planning approaches. Participants will explore how person-centred practice places individuals at the heart of decision-making, focusing on what is important to and for the person, while promoting choice, independence, and meaningful participation in their own lives.

Throughout the course, learners will experience a range of person-centred planning (PCP) tools and approaches, including practical activities that allow them to create a person-centred plan for themselves. This hands-on learning helps participants understand how planning tools can be used effectively to gather information, identify goals, and support individuals in achieving outcomes that matter to them.

The training also focuses on recognising what makes a good-quality person-centred plan. Participants will develop the ability to review and improve plans to ensure they are clear, meaningful, and reflective of the individual’s strengths, preferences, and aspirations. Emphasis is placed on ensuring plans are living documents that guide everyday practice rather than being purely administrative tasks.

Learners will gain the skills and confidence needed to facilitate person-centred planning meetings, including how to encourage participation, support communication, and ensure individuals remain central to discussions and decision-making. The course also supports participants in developing the understanding required to implement plans effectively within practice.

A key element of the session is modelling inclusive practice. Participants will explore how their attitudes, behaviours, and communication styles can promote inclusion, respect diversity, and create environments where individuals feel valued, heard, and empowered. The course aims to strengthen confidence in delivering truly person-centred support that reflects best practice and positive outcomes.

What You’ll Learn

By the end of this course, participants will understand the principles of person-centred planning, use planning tools effectively, recognise and improve the quality of plans, facilitate planning meetings, and model inclusive practice in their work.
