This course helps learners understand how to involve individuals in decisions, respect their choices and preferences, and provide support that reflects their individual needs.

Learning Outcomes

The Person-Centred Planning course provides learners with a clear understanding of the principles of person-centred approaches, including Essential Lifestyle Planning. Participants will explore how effective planning places the individual at the centre of decisions about their life, ensuring their needs, preferences, strengths, aspirations, and choices are recognised and respected.

The course provides participants with the opportunity to experience creating a person-centred plan for themselves, helping them understand the planning process from the individual’s perspective. This practical approach encourages learners to consider what is important to and important for a person when developing meaningful and achievable plans.

Learners will explore a range of Person-Centred Planning (PCP) tools and how these can be used to gather information, identify goals, and support individuals to have greater choice and control over their lives. Participants will develop the ability to recognise the characteristics of a good-quality plan and identify ways existing plans can be improved.

The training also focuses on developing the skills required to facilitate effective person-centred planning meetings. Learners will explore how to encourage participation, involve relevant people appropriately, manage discussions, and ensure the individual’s voice remains central throughout the planning process.

Participants will develop confidence in facilitating and implementing person-centred plans, including translating agreed goals and outcomes into meaningful actions. The course also highlights the importance of regularly reviewing plans to ensure they continue to reflect the individual’s changing needs, preferences, and aspirations.

Throughout the course, emphasis is placed on modelling inclusive practice, promoting independence, respecting individual choice, and ensuring people are actively involved in decisions that affect their lives.
