Learning Outcomes
The Understanding Reablement course provides learners with a clear understanding of the background and purpose of reablement and its role in supporting individuals to regain independence and confidence. Participants will explore what reablement means, how it differs from traditional models of care, and how it focuses on enabling people to achieve the best possible outcomes through person-centred support.
The course examines factors that can empower or disempower individuals, helping learners understand how attitudes, communication, and approaches to care can influence independence, motivation, and recovery. Participants will explore both the physical and psychological barriers that may impede recovery, recognising the importance of identifying and addressing these challenges to support successful reablement.
Learners will examine the benefits of following a person-centred model of care, understanding how involving individuals in decisions about their support promotes choice, dignity, and active participation. The session also introduces Prochaska’s Model of Change, helping participants understand the different stages of behaviour change and how this model can be used to encourage motivation and support individuals throughout their reablement journey.
The training highlights the importance of accurate and detailed record keeping, ensuring progress, goals, and outcomes are documented clearly to support continuity of care, effective communication, and ongoing evaluation. Throughout the course, emphasis is placed on empowering individuals to maximise their independence while delivering high-quality, person-centred support.
By the end of this course, participants will understand the principles of reablement, recognise factors that empower or hinder recovery, apply person-centred approaches, understand Prochaska’s Model of Change, and maintain accurate records to support effective reablement outcomes.
